Memberships

Important: This Vision Membership Plan is not insurance. It is not a vision care plan, discount plan, or managed care program. This plan is a direct membership agreement between the participating patient and the participating optometry practice. The plan provides members with access to professional eye care services and materials at pre-established membership pricing in exchange for a recurring fee paid directly to the practice. All prices listed are excluding additional processing fees. The membership does not make payments to any third-party payers, and it may not be combined with vision insurance, medical insurance, or any other third-party benefits.Membership fees and benefits are determined solely by the practice. Plan participation may be canceled at any time according to the practice’s membership terms and conditions. Any services or materials not covered under the membership plan are the responsibility of the member and will be billed directly by the practice. This plan is designed to provide affordable, transparent, and direct access to eye care without the limitations, pre-authorizations, or network restrictions associated with traditional insurance coverage.
